Before You Print
Rider is age 4 or older
Rider has sufficient head and neck control for safe participation

How to Register
Carefully review the Participant Information Packet
Submit Application
Participant Application Form. Return completed forms to: Riding Unlimited P.O. Box 1479 Ponder, TX 76259
Receive Confirmation
After we receive your application, the Program Director will email pre-filled forms for you to sign, date, and return.
Physician’s Referral
Download the Equestrian Rider Physician’s Referral Form. This form must be filled out and signed by your physician and sent directly to us from the physician’s office.
Final Form Submissions
Form Submission: All forms, including the Physician’s Referral, must be returned before enrollment is confirmed.
Annual Registration
Registration for all riders must be completed annually.
Cost Should Never Be a Barrier
Financial assistance forms are included in your download packet – no separate application needed. We work with every family to make therapeutic riding accessible.
